The 2025 Reimbursement Shift: Are You Maximizing Your CCM and RPM Revenue?

The 2025 Medicare Fee Schedule brings new APCM G-codes, G0511 discontinuation, and updated CPT rates. Here is a complete breakdown of every code, rate, and compliance requirement your practice needs to know.

The healthcare landscape is constantly evolving, and staying ahead of reimbursement changes is crucial for the financial health of your independent primary care practice. As we enter 2025, significant shifts in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) reimbursement are here. Are you prepared?

Understanding the 2025 Reimbursement Landscape

Discontinuation of CPT Code G0511 One of the most significant updates is the discontinuation of CPT code G0511, previously used by Rural Health Clinics and Federally Qualified Health Centers. Effective January 1, 2025, CMS no longer allows this bundled code. FQHCs and RHCs have a grace period until June 30, 2025, after which the switch to individual CPT codes for CCM and RPM is mandatory.

Introduction of New APCM Codes A notable addition to the 2025 reimbursement landscape is the introduction of new Advanced Primary Care Management (APCM) codes:

  • G0556: ~$15.20 per month for patients with one or more chronic conditions
  • G0557: ~$48.84 for patients with multiple chronic conditions
  • G0558: ~$107.07 for Qualified Medicare Beneficiary (QMB) enrollees with multiple chronic conditions

APCM can be billed in the same month as RPM but cannot be billed concurrently with traditional CCM.

2025 CCM and RPM CPT Codes and Rates

| CPT Code | Description | 2025 Rate | |----------|-------------|-----------| | 99490 | 20 min of non-complex CCM per month | ~$60.49 | | 99439 | Each additional 20 min of non-complex CCM | ~$45.93 | | 99491 | 30 min of complex CCM by physician | ~$82.16 | | 99487 | First 60 min of complex CCM | ~$134.15 | | 99489 | Each additional 30 min of complex CCM | ~$72.23 | | 99848 | 20 min of BHI care per calendar month | ~$61.79 | | 99453 | Initial RPM setup and patient education | ~$19.73 (one-time) | | 99454 | Monthly RPM device supply | ~$43.02 | | 99457 | First 20 min of RPM services | ~$47.87 | | 99458 | Each additional 20 min of RPM services | ~$38.49 | | G0556 | APCM — 1+ chronic condition | ~$15.20 | | G0557 | APCM — multiple chronic conditions | ~$48.84 | | G0558 | APCM — QMB with multiple conditions | ~$107.07 |

Note: Actual reimbursement rates may vary by region.

Maximizing Your Revenue in 2025

  • Adapt Billing Practices: If you are an RHC or FQHC, prepare for the mandatory switch from G0511 to individual CCM and RPM codes by July 1, 2025.
  • Evaluate APCM Codes: Determine if the new APCM G-codes offer a more suitable pathway for specific patient populations — especially those with only one chronic condition now eligible for G0556.
  • Stay Informed on Telehealth: Keep abreast of the evolving telehealth landscape to ensure continued compliance and access to remote care options.
  • Optimize Documentation: Ensure accurate coding and thorough documentation of all CCM and RPM services to maximize reimbursement and avoid compliance issues.
