Patient Bill of Rights

Weight-Loss Consumer Bill of Rights — Florida Statute §501.0575

Your Rights Matter to Us

iScript.care is committed to transparent, physician-led, patient-first care. This Bill of Rights outlines the statutory and clinical rights of all patients participating in iScript.care weight management programs, in accordance with Florida Statute §501.0575 and our broader commitment to ethical telehealth.

A

Rapid Weight Loss Warning

WARNING: RAPID WEIGHT LOSS MAY CAUSE SERIOUS HEALTH PROBLEMS. Rapid weight loss is defined as weight loss of more than 1½ to 2 pounds per week, or weight loss of more than 1% of body weight per week after the second week of participation in a weight-loss program.

B

Consult Your Physician

CONSULT YOUR PERSONAL PHYSICIAN BEFORE STARTING ANY WEIGHT-LOSS PROGRAM. iScript.care strongly encourages you to involve your primary care provider in any medication-assisted weight loss journey.

C

Long-Term Weight Loss

ONLY PERMANENT LIFESTYLE CHANGES — such as making healthful food choices and increasing physical activity — promote long-term weight loss. Medications are a tool, not a standalone solution.

D

Provider Qualifications

QUALIFICATIONS OF iScript.care PROVIDERS ARE AVAILABLE UPON REQUEST. All clinicians in our network are independently licensed, board-certified, and credentialed. Contact us at any time to verify provider credentials.

E. You Have a Right To:

1

Ask questions about the potential health risks of this program and its nutritional content, psychological support, and educational components.

2

Receive an itemized statement of the actual or estimated price of the program, including extra products, services, supplements, examinations, and laboratory tests.

3

Know the actual or estimated duration of the program.

4

Know the name, address, and qualifications of the clinician or specialist who has reviewed and approved the program.

5

Receive care from licensed, credentialed healthcare professionals who retain full clinical independence and are not directed by iScript.care in their clinical decisions.

6

Withdraw from any program at any time without penalty to your access to future care.

7

Have your protected health information handled in accordance with HIPAA and our Notice of Privacy Practices.

8

File a complaint with our compliance team or the U.S. Department of Health and Human Services if you believe your rights have been violated.
